Mission: Provide programs and services for older adults in cameron, elk and mckean counties including: information and referral services; perform assessments on older adults and provide care plans for adults staying in their homes safely and independently through family caregiver support, options and aging waiver programs; provide nutrition and primetime health programs in 13 senior centers; investigate reports of abuse, neglect and exploitation of older adults; provide insurance counseling and nursing home transition services
Programs: Human services programs: we supported 12 older adults and funded 718 units of service in a community adult day service programwe completed 962 assessments on 1,028 older adults we provided 51,869 congregate meals to 1,248 older adults in 13 senior centerswe reimbursed 58 older adults for 595 units of respite and supplieswe served 6,162 home delivered meals to 46 older adultswe provided 25,212 calls of information and assistance to various individualswe transitioned 25 older adults from nursing homes back into residenceswe supported 5 consumers/older adults in 4 domiciliary care homesprotective services - we took 301 reports of need and investigated 158 reports of abuse, neglect or exploitation of older adultssenior center services - we provided 909 persons with health programs in 13 senior centerswe reimbursed 64 older adults with 440 units of personal emergency response system installation and monthly fees
personal care:we funded 1284 hours of medicine management to 63 older adults in their homeswe supported 281 older adults with 46,041 hours of personal care in their homes
care management:we managed the care of 437 older adults with 520 hours of care management in our options and family caregiver programswe managed the care of 176 older adults in the aging waiver care program.
