Mission: To service older adults and certain persons with disabilities by coordinating services that address their needs and that support their independence, dignity and quality of like. And strive to be an effective advocate for our community.
Programs: Coordinated purchases of senior in home services through title iiib, care management, tcm, mi-choice waiver program, mmap, other grants and contracted senior services for 10 county area. Emphasis placed on managing chronic conditions such as pain, diabetes, preventing falls and supporting caregivers. Outcome data collected through grant activities has demonstrated these workshops positively impact overall health. During the fiscal year, 378 older adults partook of these offerings.
