Mission: The goal of the new york care coordination program is to transform health care systems to become more responsive to the interests of consumers, ensure access to high quality services and promote recovery.
Programs: Health homes of upstate new york- the new york care coordination program provides project management and back office services for four nys designated medicaid health homes, known collectively as health homes of upstate new york (hhuny). These four entities provide health home services in 22 counties in new york state and contract with approximately 60 care management agencies to provide care management services to ensure everyone involved in an individual's care is working together and sharing information that is important in supporting a person's recovery. A health home care manager is expected to help coordinate medical, mental health, and substance abuse services, and to support the social services needs of the individual as well.
children's health homes of upstate new york - the new york care coordination program provides project management and back office services for the nys designated medicaid health home known as the children's health homes of upstate new york, llc (chhuny). Chhuny includes a network of more than 100 child and family care management agencies across 54 of 61 counties in new york state. Chhuny is meeting the unique needs of the children, families, care management agencies and network partners. Chhuny is using a model of care management based upon the belief that every family has strengths and that families are the natural and most effective drivers of their care. This model also ensures that families have ''voice and choice'' at every stage in the care management process.
health home care management training and consultation - this program provides training statewide for the transition from targeted case management to health home care management, including the use of person-centered, recovery-focused approaches for care management and the supporting behavioral health, physical health, and community support systems. Funding is provided by contracts with state-wide organizations and individual providers and counties. This program also provides consultation services related to successful implementation and operation of nys designated health homes, as well as the subcontracted care management agencies which provide direct, on the ground services for the designated health homes.
