Programs: Client services: model cities provides a full range of comprehensive and culturally competent client support services. Family support services (fss) individuals and families are connected with a network of community-based partners to assist in rebuilding their lives and unifying their families. Individualized coordination of care is offered to children diagnosed with severe emotional disorders and families at risk of having parental rights terminated. Refer to schedule o for further information. Fss services includes the following: families first crisis intervention (ffci) - ffci is an intensive home-based program that serves families in crisis. Efforts focus on keeping at-risk children in their home or assisting with transitioning children back home from out-of-home placement. Case managers spend thirty-five hours per week face-to-face with family members. Home-based case management services are provided; referrals to family counseling, conflict resolution, communication skills, and support with basic needs, etc. In 2017, 9 households were served (33 individuals). Children's mental health case management (cmh) - cmh is a targeted case management program designed to assist severely emotionally disturbed children and their families to plan and coordinate needed mental health and support services. Families served receive support with assessment of child's needs, development of a case plan and connecting with resources and services. In 2017, 38 households (40 individuals) were served. Family community partnership (fcp) - fcp involves case management services that will provide assistance with family emergency funds and identify community resources while at the same time strengthen the family relationships with their children. Fcp programs served a total of 24 households (64 individuals) in 2017. Families first supportive housing (ffsh) - ffsh provides permanent housing to homeless families with one or more members enduring a chronic disability and/or the head-of-household is recovering from chemical addiction. Sankofa supportive housing provides permanent housing to young adults ages 17-25 years old who are either pregnant or parenting a child under the age of three years old. Case management services were provided to 53 households (138 individuals) in 2017. This included providing case management, financial budgeting, mental health and wellness, parenting support and empowerment. Model cities also provides services to youth. Programming provided though our youth services department consisted of the following services: juvenile detention alternative initiative (jdai) - the jdai program is designed to improve public safety and long-term outcomes for juveniles in ramsey county by reducing the number of juveniles in detention and developing appropriate alternatives. In 2017, through our community coaching programs in both ramsey and dakota counties, services were provided to 107 youth from the ages of 10-19 who were referred through ramsey and dakota county corrections. Community coaches worked with youth referred to the program on a weekly basis to fulfill their probation obligations and to connect to pro-social activities. The family coaching program serves as a short-term referral source for schools that have children who are at risk of academic failure due to poor school attendance and chronic absenteeism. Trained individual workers, referred to as "family coaches" monitor, support and link families to culturally specific community-based services, mental health services and activities that help them prevent crisis and learn how to successfully navigate challenging life situations that impact their children's ability to learn. In 2017, a total of 16 families were served through this newly piloted program. Homeownership services: model cities provides both homebuyer education and pre-purchase counseling services to families who need support with learning how to successfully navigate the homebuying process. The home stretch workshop is an 8-hour educational course offered in a day-long session on weekends and weekdays to accommodate a variety of schedules. In addition to information on choosing the right mortgage, avoiding scams and preparing for a closing, course materials are delivered in an interactive way that incorporates discussion, multi-media, additional resource materials, and presentations from volunteer professionals who have experience in housing, lending and real estate. In 2017, model cities provided homebuyer education services to 178 households. Pre-purchase counseling is designed to assist households with understanding barriers to becoming a homeowner. The homeownership advisor meets one-on-one with households to analyze their income, credit, debt and work history in order to map out a strategy to overcome any homeownership obstacles. This might include referrals to professional resources that will be able to assist in achieving homeownership including mortgage lenders and other real-estate professionals. In 2017, model cities provided pre-purchase counseling services to 74 households.
facilities management: facility management activity involves 21 units in the families first program (mcff, llc) and 16 units in the sankofa program (mcs, llc). The brownstone building (a mixed-use building(residential/commercial) was redeveloped between 2016 and 2017, and occupied by staff in october 2017. Mcff, llc provides housing for homeless families with one or more members enduring a chronic disability or chemical addiction. Mcs, llc provides housing for youth between the ages of 17 and 25 who are either pregnant or parenting a child under the age of 3 years old. All buildings are covered with an agreement with st. Paul public housing authority through its section 8 voucher or project based programs.
