Mission: Milestones area agency on aging provides and coordinates home and community based services to elderly individuals within a 17 county area in southeast iowa. The counties include appanoose, clinton, davis, des moines, henry, jefferson, keokuk, lee, louisa, lucas, mahaska, monroe, muscatine, scott, van buren, wapello, and wayne. Services are designed specifically to address nutritional and supportive servcies for individuals over 60 years of age to help them remain in their home and community.
Programs: Home delivered meals are designed for the homebound individual. The delivery of this nutrionally sound meal may be invaluable to allowing an individual to remain in his or her home. Nutrition education is also provided. Congregate meals provide a nutritious meal and reduces the isolation of older persons by combining food with friendship, socialization, and recreation. The program is available to persons 60 years of age and over and their spouses regardless of age. Nutrition education is also provided.
case management is a service that assists individuals to live in their own homes and delay (or avoid) institutionalization through home and community based services. A case manager visits the client to assess their needs and coordinates needed services with local providers.
