nhs school woodhaven is a licensed private academic school that services students within the philadelphia area with intellectual and developmental disabilities. During the fy '17, the school successfully provided educational and behavioral services to seven students. Out of the students who were discharged, none of them (0%) went to a higher level of care; the students graduated from the program successfully. This number is significantly below the pennsylvania department of education's (pde) target of a district (or school entity) sending less than 4% of their population to a higher level of care by the year 2018. Woodhaven's school exceeded this expectation a year before the target for the state of pennsylvania. Pde set out a target of 100% of students with iep's to meet "educational benefit, or make progress towards their iep goals by 2018. By the year 2017, nhs school woodhaven's students have once again reached the state's target one year early of 100% of students making progress towards their iep goals. Nhs school woodhaven continues to provide quality educational services to students with intellectual and developmental disabilities moving into fy '19, and strives to continue meeting and exceeding state/industry standards of key progress indicators.
nhs woodhaven provides residential, day and pre-vocational services to 88 adults with intellectual and physical disabilities and dual diagnoses as a licensed intermediate care facility for individuals with developmental disabilities. Services are directed and driven by annual service plans developed by the care team to include family, medical and healthcare professionals, direct support professionals, as well as other clinical and health care professionals using assessments of observable and quantifiable skills. Areas including but not limited to, activities of daily living, community interaction, safety awareness, money awareness, medication administration, prevocational development and leisure activity development are targeted through approved, documented, measurable plans and implemented throughout the year.
