Mission: Outreach/enrollment Outreach to individuals to provide information, enrollment, retention and troubleshooting of health programs and services for over 9,000 people
Programs: Welcome baby: pre and post natal home visitation program to promote health and well-being, enhance nurturing, increase breastfeeding, ensure consistent healthcare, address perinatal depression and enhance community networks of services and foster community relationships.
outreach/enrollment: outreach to individuals to provide information, enrollment, retention and troubleshooting of health programs and health services for over 18,000 people.
training: health related and social services classes/trainings and technical assistance program including affordable care act to over 1,300 people representing 200 government, social service agency, school and clinic personnel. In addition, we provide assistance applying for and renewing calfresh (food stamps), maintain weekly health education classes, a breastfeeding support group and self-support/sufficiency development arts collective to mostly our low-income clients with child care provided.
dental health for pregnant women: help to ensure that dental providers for medi-cal recipients know of pregnant women's eligibility and assist individual pregnant women to access benefits. Policy/advocacy: effect changes in health systems for low-income women and families through administrative changes in health policy and state and local levels. Examples: reviewing documents and commenting to help to ensure the enrollment system for low-income pregnant women is as simple and accurate as possible. (tce health reform grant)
