Programs: Residential - provides assistance to individuals living on their own, with family members, or transitioning to independent living, supports adults with developmental disabilities and acquired brain injury in a network of community residences providing 24-hour staff support the program operates 365 days a year and served 107 clients in the current fiscal year.
community & family supports - the community and family supports division provides customized supports to individuals living on their own or with family members. Within this division are the following programs: shared living, adult foster care, acquired brain injury, independent living and individual supports. The program operates 365 days a year and served 126 clients in the current fiscal year. Shared living and adult foster care programs place individuals who meet the eligibility criteria with families to receive services 24/7. Supports are tailored to meet each persons unique need. Families are trained in all need areas to ensure the individuals receive optimal care and supervision while placed in the home. Acquired brain injury, independent living and individual supports provide a wide range of services to the individuals within the program. All services are based upon a comprehensive assessment and are driven by consumer input and interest. These services range from medical supports, financial management and assistance, nutritional education, behavioral supports, banking, grocery shopping, adl supervision and community recreation.
day programs - day habilitation - provides therapeutic, life skills, and community integration services for individuals with disabilities in three program locations as well as services for eligible individuals residing in area nursing homes. Supported employment - provides job training and placement services for individuals with disabilities in one program location. The program operates 260 days a year/5 days a week and served 20 clients in the current fiscal year.
