Causes: Hospices, Human Services
Mission: To ensure the highest quality of life possible for individuals and their families facing a life-limiting illness, to provide bereavement support to individuals, and to promote an understanding of hospice and palliative care services.
Programs: Patient care program - high quality palliative and hospice care and support to individuals and their families who are facing a life- limiting illness, including visits by nurses, hospice aides, social workers, spiritual care coordinators and volunteers. Clients served - 246 hospice clients and 59 palliative clients. Admits included 38 carry over hospice patients and 208 new patients, 10 carry over palliative patients and 49 new patients. Days of care provided - 14,483.
bereavement and outreach awareness programs - support to hospice and palliative families and the general community, and promoting an understanding of hospice services. Fundraising/awareness events include concert in the sky, three candle-lighting memorials, and trees of remembrance.
