Mission: To advance and promote excellence in end of life care by: advocating for its members, patients, families and the end-of-life care giving community; providing education and enhancing awareness.
Programs: As a result of hpcfm's continued collaboration with the department of health care quality on section 277 of the law which mandates certain health care providers to inform patients about palliative care, hospice care, and end of life options, the department of public health released know your choices: a guide for patients with serious advancing illness. Hpcfm promoted excellence in end-of-life care via the delivery of 6 educational opportunities, including a 3 day conference in october, attended by over 500 end-of-life care professionals; a one day conference on hospice item set (his); icd coding workshops and hospice health aid training; and provided 24 webinars for hospice and palliative care professionals. Hpcfm attended over 30 legislative meetings on beacon hill and capitol hill to promote excellence in end-of-life care! Including han's advocacy intensive with representative from hpcfm member hospices. Hpcfm collaborated with african doctors to promote palliative care in conjunction with nhpco's global partners.
