Programs: Visiting nurse of hopehealth (vnhh) provides high quality health care services designed to promote and maintain community health and to provide in-home health services for patients requiring health care assistance in the home. In fy 17, vnhh provided nurses, home care aides, social workers, physical, occupational and speech therapists services to 4,500 patients and families in the comfort of their homes. Founded in 1908, vnhh's mission, to improve the quality of life in the communities it serves, remains unchanged. In addition to skilled nursing care for common home health needs such as post-operative care, wound care, iv therapy, and physical, occupational and speech therapies, the agency offers remote telehealth monitoring for heart failure and other patients with health conditions. The agency also offers a palliative home care program designed to manage the complex medical needs of seriously ill patients. Vnhh medical social workers identify the non-clinical barriers impacting successful patient outcomes and provide assistance in long term planning and in the coordination of community recourses. All of these programs are offered to patients regardless of their insurance coverage. The organization has nurses, therapists, social workers and home care aides known throughout the region for the expert and compassionate care they provide. Vnhh clinicians are available 24 hours a day and respond readily to urgent patient needs. The agency has adopted the highest standards in home care, as validated by its accreditation from the the joint commission. Beyond services provided to vnhh's patients and families, members of the agency's leadership and staff provided community leadership in a variety of ways from serving on local not-for-profit boards and participating in other community and health forums.
