Programs: Home health services (hhs) of wjcs provided approximately 135,000 hours of personal care to people in need including a specialty in traumatic brain injury and nursing home transition and diversion during the year ended 6/30/17. Personal care aides and certified home health care aides, under the supervision of registered nurses, provide personal care services to individuals who are unable to perform personal care services including (but are not limited to): bathing, grooming, dressing, ambulation assistance, transferring, toileting, meal planning, feeding, cooking, marketing, medication reminders, range of motion exercises, light housekeeping, assistance negotiating the community and socialization. The primary focus of the program is to maintain community living in a manner that is safe and preserves the dignity of the client and family.
