Mission: Cultivate partnerships and deliver quality health care to strengthen rural communities.
Programs: Medical and mental health services: health care collaborative of rural missouri (hcc) owns and operates 4 community health centers in rural missouri that provide medical, mental health and dental services. In 2016 we served 4,907 unique patients. 2,629 patients were seen for medical services resulting in 7,438 medical visits. 167 patients were seen for mental health services resulting in 455 mental health visits.

By cultivating partnerships with multiple entities in our surrounding areas we are able to strengthen rural communities. Through network memberships, consulting services, community outreach events and our warehouse we are able to reach deeper to treat the whole person and break through the socioeconomic barriers that exist in rural communities.
