Engage local and state organizations in developing, translating, and disseminating programs that improve the health status of older adults and their caregivers. Work to advance the mainehealth accountable care organization (mhaco).
matter of balance training and program: disseminate, train and support the matter of balance program at the national level.
the post-acute care work is an outgrowth of the senior living collaborative (nursing home) efforts that have been underway for a long-time. This focus is expanding to include home health, hospice, and community services to better serve patients and families as they move across the continuum of care.
