Programs: Home health and rehabilitation services are specialized in-home medical services that have been ordered by a physician for a homebound patient after surgery or diagnosis. The services are typically short term geared to facilitating the patients recovery and return to independence as efficiently as possible. Fayette home care offers a variety of in-home skilled nursing care services including wound and dressing care, diabetic care, administration of injections, maternal/child care, etc. 23,952 skilled nursing visits were performed during fiscal year 2012. Rehabilitation services help a patient restore certain functions and abilities. Physical, speech and occupational therapists are available to fit patient needs. 12,719 therapeutic visits in physical, speech and occupational therapy were performed in fiscal year 2012.

hospice program provides care for those in the last stages of life - people who wish to remain in the warmth and familiarity of home. Medical attention is provided to patients with life-limiting illnesses. The program is designed for individuals with life expectancy of less than six months. The patients personal care is provided by a home health aide who is supervised by a registered nurse. Medical equipment and necessities are supplied at no cost, if needed dietary counseling and physical, occupational and speech therapies are available. 235 patients were provided 14,281 days of hospice service during fiscal year 2012.

diversified services provides registered nurses and licensed practical nurses, certified home health aides, sitters and housekeepers to enable people to continue living independently. Fayette home care staff coordinates with the clients physician to develop a plan of care, and their registered nurses will perform evaluations as necesary. 13,558 hours of service in the therapy, and medical social services were provided to 96 patients during fiscal year 2012.
