Causes: Mental Health
Mission: Evergreen foundation strives to improve access to and public awareness of high quality prevention, treatment and support services by the provider community to individuals and families with intellectual/developmental disabilities, behavioral health, and/or substance abuse needs in cherokee, clay, graham, haywood, jackson, macon and swain counties. Activities include:i. Infrastructure support to the service networkii. Start-up and service provision grantsiii. Scholarships to enhance the quality of the provider networkiv. Public awareness/educationv. To support evergreen foundation activitiesvi. Advocacy on behalf of consumers and the provider community
Programs: 1. ) issued grants to: a. ) 30th judicial district domestic violence sexual assault alliance, inc. (501-c-3 organization) in the amount of $6,000 for planning, development and financial viability of a therapeutic kennel project in wnc to train service dogs, therapy dogs and create a revenue generating kennel. B. ) arc of haywood county, inc. Inc. (501-c-3 organization)in the amount of (i) $24,000 to provide funding for the continuation and expansion of the community living program serving adults with developmental disabilities in haywood county. (ii) $6,200 to provide for recreational and social activities for individuals with developmental disabilities in haywood county. Coordination of events, consumer travel, supplies and venue rental were covered through the grant. (iii) $3,150 to replace 11 computers, most of which are over 10 years old. C. )meridian behavioral health services, inc. (501-c-3 organization) in the amount of (i) $47,450 to provide for the continuation of the patient assistance program in haywood and jackson counties whereby indigent consumers are assisted in obtaining their prescriptions at no cost and to expand the program by another . 50 fte. (ii) $30,000 to provide for the continuation of jail assessments and group therapy services in haywood and jackson counties. (iii) $11,000 to replace a high mileage van for the psycho-social rehab program in haywood county. Program serves 30-35 members daily providing transportation to and from the program on a daily basis. (iv) $20,000 to obtain an independent business valuation of jackson county psychological services as part of the due diligence process as the two organizations pursue a consolidation of the two entities. D. ) mountain projects inc. (501-c-3 organization) in the amount of $6,500 to provide for the western nc summer teen institute program, a 3-5 day program designed to train, mobilize and empower youth to prevent the illegal use of alcohol, tobacco and other drugs. E. ) shared vision consulting, llc. (s-corporation) in the amount of $18,370 to develop a sexual abuse intervention network (sain) addressing child sexual abuse prevention and intervention across the seven wnc counties. F. ) appalachian community services (s-corporation) in the amount of (i) $26,244 to provide for an alternative service to the state funded community support team service eliminated by the lme/mco 8/23/12. (ii) $7,500 to provide transportation services for voluntarily committed consumers upon discharge from psychiatric hospitals, to meet the discharge plan requirements. G. ) h. A. V. E. N. Children's advocacy center (501-c-3 organization) in the amount $8,000 to provide for the provision of child forensic interviews in cherokee, clay and graham counties and to obtain the cornerhouse training in forensic interview protocol for up to 30 providers in cherokee, clay and graham counties. H. ) webster enterprises, inc. (501-c-3 organization) in the amount of $9,750 to expand their sewing facility to create up to 15 new positions for people with disabilities or disadvantages and increase their revenue by $800,000 annually. I. ) jackson county family resource center (501-c-3 organization) in the amount of $3,500 to provide case management services for the homeless shelter in jackson county during the winter months to enable individual who are homeless or at risk of being homeless to navigate the mental health system. J. ) jackson county psychological services, pa. (s-corporation) in the amount of $11,255 to provide partial funding to purchase three vans for day treatment programs in haywood, jackson and macon counties. K. ) kids advocacy resource effort, inc. (501-c-3 organization) in the amount of $12,500 to provide a victim advocate program addressing abuse and neglect of children and youth in haywood county. 2. ) leased apartments to 4 developmentally disabled adults at less than 33% of fmv of comparable triple net lease cost in the area, assisting these individuals to live independently. 3. ) leased homes to 2 alternate family living providers caring for two developmentally disabled adults at less than 60% of fmv of comparable triple net lease cost in the area, helping these families and individuals to live in a natural family setting versus more costly and less independent group home settings. 4. ) leased homes to 4 developmental disability providers caring for eight developmentally disabled adults at less than 75% of fmv of comparable triple net lease cost in the area, helping these individuals to live in a natural residential setting versus more costly and less independent aggregate group home settings. 5. ) leased facilities to 2 mental health providers, one serving up to 40 children a day in a day treatment program environment and the other serving up to 50 older adults in a day program helping to avoid admission to nursing homes both at less than 50% of fmv of comparable triple net lease cost in the area, helping these children to stay in school and the older adults to have meaningful social activity on a daily basis. 6. ) leased 8 facilities to mental health & substance abuse providers, serving children and adults with mental health, substance abuse issues at less than 65% of fmv of comparable triple net lease cost in the area improving access to care, throughout our seven county service region.
