Mission: Our mission is to partner with the people we serve, families, communities and other human service organizations in providing quality health care services and supports. Our vision is to empower the people we serve to pursue their hopes and dreams in partnership with their families, communities and other human service organizations.
Programs: We provide community based psychiatric rehabilitation services to seriously and persistently mentally ill adults. This includes individuals with schizophrenia, serious depression, bipolar (manic-depressive illness), panic, anxiety, post-traumatic stress, severe phobias and borderline personality disorder. Our services, include community support (provided by a community support worker), activities (educational and social activities), and medication services (psychiatrists work with the community support worker to provide the best care). We provide supported employment services to assist persons with a mental illness or developmental disability in returning to the workforce which includes finding a job suitable for the individual, job coaching and support. We served approximately 550 adults in this program.
we provide case management, medication management, community support, family support, and therapy/counseling by professional staff (crendentials in psychology, sociology, counseling, or social work) to children or youth with a mental health diagnosis. Our goal is for children and youth with a mental health diagnosis to be successful at home, school and in the community. We served approximately 350 children and youth.
we provide outpatient services that include psychiatric/psychological evaluations (conducted by a psychiatrist or psychiatric nurse practitioner for ages 3 years and up), individual psychotherapy (conducted by a licensed therapist), group psychotherapy counseling (facilitated by licensed therapists), medication management (conducted by a psychiatrist or advanced practice nurse), family and marital counseling (therapy conducted by licensed therapists), nursing support (including assistance with medication issues and patient assistance programs) and behavioral wellness education programs to the community at large. In 2014 we increased our scope of services to include primary care and dental services. We served approximately 3,500 adults, youth and children.
we operate a 16 bed transitional living, psychiatric group home. We serve adults with complex and co-occurring conditions who require assitance and support before they can successfully transition back into the community. We served 20 adults. We provide services under a healthcare home model for individuals on medicaid that need monitoring of their medical and behavioral status. The services are to improve the individuals heath status and reduce hospital admissions. We served approximately 375 adults and youth. We provide mobile and onsite 24 hour a day crisis intervention services for adults, youth and families. We served approximately 300 individuals.
