Programs: Medicaid waiver programs - passport and assisted living:passport is a community-based long-term care program for medicaid eligible older adults 60+ that provides in-home services instead of permanent nursing facility placement. Services provided may include: in-take, in-home assessments, care management, personal care, homemaker, adult day services, emergency response units, home delivered meals, transportation, minor home modifications, chore and specialty consultations for nutrition and social work counseling. Assisted living is a community-based long-term care option for medicaid eligible older adults that provides an alternative to nursing facility placement by bridging the gap between passport in-home services and permanent nursing facility placement. Residents have access to assistance 24 hours a day, one time community transitions services as well as care management. Passport and assisted living have level of care and financial eligibility requirements that must be met in order to qualify for program services. Approximate number of unduplicated members served 3,059.
mycare ohio is ohio's dual demonstration pilot with cms and the state of ohio to provide long-term care systems and supports to medicaid and medicare eligible adults over the age of 18. Services provided to eligible adults care management, care plan and service plan review, and regular face-to-face visits. Approximate number of unduplicated members served 4,424.
elder rights program - ombudsman:trained and certified ombudsman advocates promote quality compassionate care for consumers of long-term care services through direct intervention, information, education and advocacy. Ombudsman protect the rights of all consumers of care regardless of age, work to resolve care and service issues, and work to transition nursing facility residents to a setting of their choice. Assistance is provided to consumers and families regarding selection, facility performance, and guidance on rights issues. Certified ombudsman staff and volunteers provided 25,623 hours of service in the region. Approximate number of unduplicated members served 21,876.
