Mission: Dartmouth-hitchcock health (d-hh), is a new hampshire (nh) nonprofit corporation exempt from federal income taxes under section 501(c)(3) of the internal revenue code (irc). D-hh was formed in may 2009 with operations commencing on october 1, 2010. D-hh is operated for the following charitable, educational and scientific purposes: -to establish, manage, govern, and fundraise for an integrated healthcare delivery system that best serves the purposes of preventing, diagnosing, treating and curing human illness within the northern new england region; -to manage a healthcare system that provides health care services to the public in a cost-effective manner; -to establish and maintain cooperative hospital and provider relationships throughout its system; -to achieve excellence in clinical innovations, service, quality, cost and outcomes, supported by a strong academic program; and to integrate research, training, information technology and academic medicine in the provider organizations thr
Programs: Dartmouth-hitchcock health continues to work to establish and maintain cooperative hospital and provider relationships throughout the northern new england region. D-hh continues to refine affiliations with four hospitals and visiting nurse and hospice of vermont and new hampshire (vnh). The more recent affiliations with alice peck day memorial hospital in lebanon, nh, and with vnh are allowing more coordinated care and efficiencies across system, as well as more effective utilization of hospital beds and clinical services. Benevera health, a population health improvement company focused on care coordination and quality improvement, formed with harvard pilgrim, elliot health system, st joseph hospital, and frisbie memorial hospital, began operations on january 1, 2016. As of june 2017, benevera was responsible for managing the health of more than 100,000 member lives. Improved and coordinated patient care, with early intervention when appropriate, is intended to help drive down the cost of health care, allowing harvard pilgrim to keep premiums lower and consistently more competitive for members.
a major d-hh initiative to create integrated relationships across new hampshire and vermont continues. The onecare vermont aco, co-owned by d-hh with the university of vermont health network, is working closely with the state of vermont to implement the states global commitment to health section 1115 waiver from cms. As of january 1, 2016, d-hh restructured its share risk contract with harvard pilgrim health to include dartmouth-hitchcock clinic, cheshire medical center, and new london hospital. During fy 17, d-hh worked on becoming a cms next generation aco, and received approval as of january 1, 2017. Under the next generation aco model, d-hh is sharing financial risk with cms for the population health management and total cost of care of approximately 21,000 attributed traditional medicare fee-for-service beneficiaries who have a dartmouth-hitchcock clinic primary care provider in one of the southern nh practices.
philanthropy across the d-hh system is coordinated by a joint development office, working on behalf of d-hh and the geisel school of medicine at dartmouth. The joint development office also manages revenue from events such as the norris cotton cancer centers prouty race, various events sponsored by the childrens hospital at dartmouth-hitchcock (chad), and the jack byrne center for palliative & hospice care. Approximately 90% of d-hhs philanthropic revenue comes from donor contributions and event revenue. During fy17, more than $18 million was raised on behalf of d-hh through the joint development office.
