Nonprofit Overview
Causes: Health, Health Care
Mission: To make quality hospice care available to terminally ill persons, their families and those affected by death and dying in out community; to advocate effectively for patient comfort, dignity and choice; and to be recognized as a leading resource in clinical, ethical and spiritual issues of dying and grief.
Target demographics: We provide care where the patient lives, their home, nursing home, assisted living facility or in a hospital setting. The care is provided by a team of hospice professionals and volunteers who provide physical, emotional, psychosocial, and spiritual support to the patient and family. Our care team includes physicians, nurses, certified nursing assistants, social workers, chaplain, bereavement counselors and trained volunteers.
Geographic areas served: Crater Community Hospice serves families in Chesterfield, Dinwiddie, Greensville, Prince George, Sussex, Surry, and Charles City, Ashland, Petersburg, Colonial Heights, Hopewell, Emporia, Richmond and surrounding areas.
