Programs: Coalitions - we recognize that working and strategizing together is the only way to improve the future health of washington communities. We offer strategic support in the form of funds and technical expertise to support community based health solutions through the work of coalitions. The coalitions we support work related to the prevention alliance, chw, and the work of our fiscal partners (copc and cross cultural collaborative). The prevention alliance incubates pse (policy, systems and environmental) strategies for local and statewide health improvements. We coordinate this broad collaboration between cross sector groups representing transportation, healthy eating, tobacco prevention, school-based clinics, local coalitions, housing, and local health departments. Estimated impact 250,000. The childhood obesity prevention coalition (copc) was established in 2007 to provide guidance and leadership to decision makers that have the power to change our physical and social environments. Today, we claim 53 member organizations and the copc has made healthy choices easier through eating better and moving more. Total impacted: 108,768 ; the cross cultural collaborative collaboratively promotes community health and system change in formal and informal health care systems to be accessible, available and culturally appropriate for all. The cross cultural collaborative was awarded community transformation grant funds in 2013 to partner with five local community coalitions whose focus is community health. Their objective is to coordinate efforts of the activities under the grant funds within a health equity framework. Estimated impact 5,000.
tech asst. - true collective action requires technical support and expertise that prioritizes community ownership of the work while being grounded in what is known and what works. Creating and supporting active feedback and rapid learning cycles between community, leading edge science and decision makers is critical to creating enduring change. During 2013, chef provided technical assistance to local health jurisdictions and their partners around policy, systems and environmental change. Chef also worked with public housing authorities to assist them with no-smoking policy adoption for their properties, protecting staff and residents from the dangers of secondhand smoke. community health advocate is the program of frontline workers who help people and communities improve their health. Developing and deploying a cha workforce can play a significant role in helping improving overall health by building health communities and improving chronic illness care through better integration of care and social supports. During 2013, the salishan community health advocates served approximately 929 people in community kitchens, senior bingo, community gardens, youth bike club and individual support with diet/exercise/smoking cessation/managing chronic disease. In addition to holding 13 events and advocates completing 292 hours of training in addition, the community health advocates are working across generations and across cultures to build community capacity. This is in addition to chef's support through seed-money/grants in washington state communities impacting approximately 1500.
community education/healthcare reform programs are purposed to provide resources to support prevention leaders and practitioners in responding to the implementation of the health care reform in washington state in a manner that maximizes the opportunit to improve health outcomes for vilnerable populations.
