community housing network remained the largest permanent supportive housing provider in oakland and macomb counties, serving 536 people in permanent shelter housing and shelter plus care. Eight people, 6 in oakland county and 2 in macomb county, transitioned out of the leasing assistance and shelter plus care programs and are now renting independently. In addition, 1 person exiting the program became a homeowner and 33 persons exited with a rental subsidy.
community housing network is the lead homeless management information system (hmis) agency in oakland county and is responsible for the county-wide implementation. As lead agency, the hmis administrators manage and assist use of the system for 11 partner-agencies and more than 100 end-users. Projects at chn utilize hmis to input client and project data that is reported to funders and to the community at large. The system administrators work closely with partner agencies and the michigan coalition against homelessness to ensure compliance and data quality standards are met. Hmis has been used as a tool to measure outcomes and effectiveness and has supported the community in being leaders of innovation. Community programs:community housing network's housing resource center call center responded to 22,471 calls with a wide range of housing issues and questions. As part of the screening process and follow-up linkage to programs, 672 individuals/households met with hrc housing specialists for face-to-face meetings to pursue housing resources. The projects for assistance in transition from homelessness (path) team reached out to 672 homeless individuals, of which 436 were enrolled in the program after meeting eligibility criteria and providing consent. The emergency solutions grant program assisted 67 individuals in oakland county with rapid rehousing and 35 individuals with prevention assistance. The oakland county rapid rehousing grant from the u. S. Department of housing and urban development served 340 people with short-term assistance, comprised of 167 adults and 173 children. 11 preliminary meetings and 17 voucher briefings, facilitated by mshda housing agents, were held at community housing network to assist homeless households in completing their application for the homeless preference housing choice voucher. The housing counseling program provided homebuyer education to 1,124 households assisting them in the purchase of a new home. Chn's foreclosure prevention services assisted 48 households experiencing mortgage or property tax delinquency. Development:a total of 759 individuals and families were supported through the 2016 holiday season annual adopt a family program. Volunteer opportunities totaled 11,674 hours of service, the majority of which was contributed by seven americorps members and 14 interns. Getting my own address:the getting my own address (gmoa) program reached 295 people through presentations, an oakland county support group, individual counseling and networking opportunities. Since september 2011, the gmoa program has served 4,694 participants.
