Programs: Community health partners is a federally qualified health center providing primary health care services for residents of park and gallatin counties in montana. During fiscal year 2016-2017, our four medical clinics located in livingston, bozeman, belgrade, and west yellowstone, and our two dental clinics located in livingston and bozeman, served approximately 11,750 patients, providing medical, dental, and integrated behavioral health services to all patients regardless of ability to pay. Patient fees for all services are adjusted using a sliding fee scale based on federal poverty guidelines. During this period, sliding fee discounts applied to patient visits totaled $757,444. Through our onsite resource coordinators and outreach and enrollment specialists, we provided connections with community resources able to help with non-medical needs, as well as provided assistance with health insurance enrollments. Through the medication assistance program, 143 prescriptions were provided at little or no cost to patients, while 2,957 prescriptions were provided through the federal 340(b) program. We estimate that access to these pharmacy programs provided savings of approximately $270,607 for our patients during the period july 1, 2016 through june 30, 2017.
chp learning partners integrates educational programs and support services within the medical clinics, empowering individuals and families to become self-reliant, life-long learners. During the 2016-2017 fiscal year, the learning partners program provided 533 home visits with families through the parents as teachers program; helped 30 adults earn the equivalent of a high school diploma, or advance their education in pursuit of a job or post-secondary education; provided 39 families with intensive case management through the temporary assistance to needy families program; and facilitated the reach out and read program at all clinics, putting 1,437 new, age-appropriate books in the hands of young children.
