Mission: Tcf provides community-based adult day health services to adults coping with chronic medical conditions, disabilities, or alzheimers disease. Tcf is dedicated to improving the quality of life for both our clients and theirs caregivers. Tcf provides educational services and caregiver support groups the community.
Programs: Adult day health services - everett center - the everett center serves a very diverse, primarily very low-income, group of adults who have medical conditions requiring consistent monitoring and intervention. The center provides a hollistic day program consisting of nursing care, therapeutic activities, meals, social services and transportation. The center provided 15,508 units (days) of care to an average of 61 adults a day.
alzheimer's specialty adult day health care - medford and lowell centers - the medford and lowell centers are designed specifically to serve those with memory loss. Both centers have a high staff to client ratio and emphasize programming that reduces frustration and heightens self-esteem. Nursing services, social services, meals, transportation and caregiver support groups are provided. The medford center provided 8,689 units (days) of care to an average of 29 adults a day. The lowell center provided 10,331 units (days) of care to an average of 34 adults a day.
