Mission: Community care health plan's mission is to develop and demonstrate innovative, flexible, community-based approaches to care for at-risk adults, in order to optimize their quality of life and optimize the allocation of community resources.
Programs: Cchp's pace program provides a full range of long-term, primary, acute, and preventive care to enable individuals who are 55 years old or older and certified by their state to meet a nursing home level of care through the pace program. Today's fragmented health care financing and delivery system comes together to serve the unique needs of each individual to help him or her live as independently as possible in the community. The service area includes two counties in southeastern wisconsin. As of december 31, 2017 cchp's pace program enrollment was 561 participants.
the wisconsin partnership program allows participants to keep their current doctor while receiving benefits of the program. Participants receive all-inclusive health care while continuing to live in the community. Services are delivered in the doctor's office, in the participants' home. The service area includes nine counties in southeastern wisconsin. As of december 31, 2017 cchp's wisconsin partnership program enrollment was 666 participants.
