Causes: Centers to Support the Independence of Specific Populations, Human Services

Mission: To provide the resources, expertise, and commitment to assist people in meeting their needs to maintain quality of life, productivity, and independence.

Programs: Our home care program, dba trusting hands home health care, is a fully licensed medicare and medicaid provider that offers skilled nursing, licensed nursing assistant, and homemaker companion services to residents in sullivan, western merrimack, and grafton counties. In fy 14 we provided services to over 60 individuals.

our transportation program provides affordable and dependable public transportation services in sullivan county. In fy 14 we provided services through 3 major corridors and also provided demand response (dial-a-ride) services. Also in fy 14 we offered rides through a volunteer driver program that allows us to provide transportation to individuals living in sullivan county who are not able to access our other transportation services. In fy 14 we provided over 29,000 rides to individuals within sullivan county.

our family services program provides town assistance for the towns of sunapee, charlestown, unity, and lempster and provides an alternative process for first-time offenders that educates and re-directs aberrant behavior, and engages adolescent or adult offenders in meaningful learning experiences to foster personal and social responsibility, accountability, and appropriate decision making skills. In fy 14 we served over 120 individuals in our youth program.
