Mission: To provide people with disabilities and their families with the resources, services and opportunities to be contributing members of their communities and to achieve the most fulfilling and meaningful lives possible.
Programs: The organization has four day habilitation locations. In total these programs served 223 people. Southbridge day habilitation offers a wide range of skills training opportunities in a home-like setting. This program is designed for individuals who like a quieter program and are more medically fragile. This site serves as retirement home for many of the senior citizens served by the agency. West street day habilitation is designed to serve those individuals who are medically fragile and require a high level of medical, personal care and therapeutic supports. Quinebaug river day habilitation is designed to serve those individuals who have sensory integration deficits and behavioral challanges. This program has a high staff to program member ratio with many individuals receiving one to one supports. Center of hope day habilitation focuses on community membership and volunteerism. This program is ideal for individuals with a high level of energy. In addition, it provides skills training, assistance in daily living, and restorative services.
this employment supports program and the sheltered workshop served a total of 218 participants. This program provides work evaluations, employment skills training, on-the-job support services and direct job placement services to individuals with disabilities. Work options include production and assembly, maintenance, clerical, auto detailing, yard work, recycling and banner production which are all performed within the center of hope or its small businesses. Outside work includes: job placements in a wide range of community locations, janitorial services and delivery services and a vast network of volunteerism sites.
the transportation program served 400 individuals and provided morning and afternoon transportation from their homes to our programs. Transportation services include single trip transports, small group transportation in mini-vans, wheelchair transportation vans and 15 passenger group transportation vans.
