Mission: Our vision: All stakeholders in the health care continuum are aligned toward optimizing the health of populations.
Target demographics: Wellness and care management organizations, pharmaceutical manufacturers and benefits managers, health information technology and biotechnology innovators, employers, hospitals, physicians, nurses and other health care professionals, and researchers and academics.
Programs: Cca hosted its annual meeting, the forum, focusing on a very wide range of the most salient practical, regulatory, and policy issues for the population health and the wellness industry. The conference emphasized implementation of health and wellness models, and strategies from development to delivery, with a strong focus on evaluation.
cca managed several research initiatives with industry partners, including reccomending measures and standards for the assessment of population health management programs deliverd to an employers workforce, role of care coordination in the context of patient-centered medical homes, best practices for health care consumer engagement, and focus on best practices for engagement strategies.
