Mission: Bridging access to care, inc. Provides comprehensive hiv/aids, mental health and substance abuse related services to under-served racial and ethnic minority communities in the borough of brooklyn.
Programs: Homeless prevention - bac assists homeless and marginally-housed people living with hiv/aids and/or with histories of substance abuse through its scatter-site housing programs. These services include: housing placement assistant program, living skills groups, permanent supportive housing, scatter site housing, and housing and supportive services on a long term basis for homeless persons with disabilities.
wellness services - bridging access to care operates an office of mental health (omh) licensed outpatient clinic offering comprehensive services to clients who have a range of mental health problems. The programs objective is to improve health outcomes through symptom management/stabilization. The services provided include: comprehensive psychiatric and psycho social evaluations, individual and group psychotherapy, medication management, home & community based services, and other wellness support services.
prevention - bridging access to care (bac) conducts outreach activities that actively engage high risk individuals and hiv positive individuals. Bac provides enhanced service delivery to residents residing in brooklyn new york by increasing access to hiv related services, increasing retention and treatment outcomes and improving the quality and or intensity of these services. The services include youth services, adult services, gay/bisexual/trans gender/msm (men who has sex with men) services, and prevention and support services for women.
