Mission: To provide a stable foundation for children, adults and senior citizens. Case management services were provided for adults with disabilities and senior citizens 65 years or older within their communities. The agency provided support coordination services to young adults who were aging out thru the division of developmental disabilities.
Programs: The organization was looking to start a hospice program our expenses included; application fee of $2,000 plus state training $2,318. 79, fingerprinting $715. 10, and background check fees of $162. 80. State training was required for license. We were not able to move forward with the program because we were not able to obtain full participation from the required parties which included; a pastor, doctor rn, social worker, admin, home health aides and infection control person.
the organization held a tricky tray fund raising in a effort to cover the expense for the hospice statrup. Items were purched to use for biding. Although the event was not well attended we were able to raise $1000
