In 2017, bsfhc served 15,415 patients through 66,000 patient visits at two clinics and four school-based clinics. As a federally qualified health center, bsfhc plays a vital role in providing health services to poor and medically underserved populations who often have difficulty accessing quality health care.
wic program:our mission is to work diligently with the new york state wic program to reduce and prevent obesity by providing culturally sensitive participant-centered nutrition and breastfeeding education. We strive to serve the participants with dignity and to refer them to appropriate health care and relevant services that are beneficial to them. Our vision is to help low-income women deliver healthy babies and raise healthy children. Description:bed-stuy wic, a special supplemental nutrition program for pregnant, postpartum/ breastfeeding women & children up to age 5. We provide help for nutritious foods and nutrition education. We promote a healthy lifestyle, breastfeeding, as well as make referrals to health care and other community services. For 2017, wic provides services to approximately 2,300 individuals.
msa wellness program:the wellness center services are to assist patients and support providers on specific needs of the hiv population in primary care within the health center. The goals for the program are retention in care, which includes all aspects of the patients health needs. Make referrals and link patients to services not provided by the health center. We track and monitor patients for medical compliance at all levels. We meet with providers to discuss patients' health and other needs that will retain patients' care, while addressing the patients' problems concerns. For 2017, wellness enrollment was 141 primary care patients. The wellness program generated more than 1800 encounters in 2017.
healthy families successful start:successful start is a healthy families new york program based on the healthy families america model. It is an evidence-based home visiting program that offers services to expecting parents and new families, beginning weekly and decreasing over time until the child starts school or head start. The program is funded by the office of children and family services. The goals are:- to prevent child abuse and neglect- promote positive parenting skills and parent child interaction- ensure optimal prenatal care and child development- enhance family self-sufficiencythe program serviced a total of 80 families, with 734 home visits completed during the july 2017 - june 2018 contract period. Performance targets:- 67% of families received a parental stress screening immediately after birth. - 100% of enrolled families breastfeed child for at least 3 months. - 100% of children were assessed for the risk of lead in their environment. - 100% of children demonstrated age appropriate developmental milestones on the ages and stages questionnaire and referred for further evaluations/services if delays are detected. - 100% of children and primary caretaker had medical providers.
