“The Back on Track Program” Henry Tarkington has over ten years of fund raising experience. He has worked a wide range of institutions from grassroots to national organizations. Every day we are inspired by the countless ways our clients engage in philanthropic work. It is our great honor to respond to your interest. Just as we’ve been honored to have helped many for-profit and non-profit organizations in California raise hundreds of thousands of dollars a year in funding support towards funding goals since our founding in Program Year 2000. The achievement is not ours to claim, of course. As thoughtful guides and counselors in your granting campaign, our role is to elevate and focus your journey toward the abundant outcomes intended for you. Back on Track Veteran Center cosist of training and job development programs since year 2000. Our goal is to build the capacity of the Center to meet the growing needs of veterans in the Inland Empire. We believe that it is in the best interest of the Department of Veterans Affairs and the community. 1. To have many strong, well developed effective, nonprofit organizations. 2. To collaborate with as well as to achieve its philanthropic goals. 3. To build, the capacity of this nonprofit agencies charitable dollar. 4. To make it more effective in solving this growing communities needs with its programs. Homeless Shelter provides a full continuum of care, a comprehensive program with five basic services: 1. Emergency Shelter: (ES) of 29 beds for adult men and women 2. Transitional Housing: Tarkington House two oceanfront apartments for (ES) graduates. 3. Mental Health Outreach: workers seek out mentally ill who are homeless on the streets. 4. Employment Development: to help homeless persons return to the world of work 5. Day Treatment Program: for the homeless mentally ill. Tarkington House Homeless Shelter is unique because it operates as a homeless shelter and a rehabilitation center. Our mission is to close the revolving door nature of homelessness. There are five basic programs 1. Emergency Shelter: The shelter provides housing, food and clothing to 29 homeless men and women. A case manager assigned for each resident helping the client set realistic goals, to transition to self-sufficiency. Residents participate in individual and group counseling provided by interns supervised by program Director. Life skills, self-enhancement workshops, and health education workshops are offered. Residents also receive health evaluations that include AIDS, HEP-C, T.B. testing,Tramatic Brain Injury, Post Tramatic Stress Disorder. Residents participate in drug and alcohol programs to attain and maintain sobriety, on-site A.A. and N.A. meetings. 2. Transitional Housing: Tarkington House is our transitional housing program. It is located in two apartment buildings on the Oceanfront. Ten shelter “graduates” transition from the emergency shelter into each apartment building. Where they pay low rent and continue to work on their long term goals related to employment and self-sufficiency. They gain stability in their lives as they maintain their sobriety. Their employment enables them to save money for future independent housing. Tarkington House provides an opportunity for the homeless to build upon their strengths. The program provides 180 day follow-up care with case management providing support. 3. Homeless Shelter. Residents participate in weekly “community meetings”; they take responsibility for chores around the complex. They must abide by strict no drugs and no alcohol guidelines. 4. Mental Health Outreach: Day Treatment Program for Homeless Mentally Ill: One Stop Case Management and Activities for mentally Ill. Medical assistance is also available on request. The agency’s mental health outreach worker seeks out the mentally ill homeless on the streets. They help them connect with appropriate resources within the community. The outreach worker seeks out locations where the homeless mentally ill congregate. They respond to calls from hospitals, private businesses, social service agencies and private citizens. Homeless shelter provides ten beds exclusively for mentally ill homeless persons. The agency operates a day treatment program for the mentally ill residents including counseling / life skills workshops. 5. Employment Development: Homeless Shelter staff provides assessment, job readiness workshops and job placement services. The agency participates in the Job Training Partnership Act, which provides a variety of job training opportunities. Through the Workforce Investment Act and One- Stop Career Centers, linkages provided to classroom. Full time employment placement, and on the job training programs can be found at a one – stop. The Shelter maintains a fully equipped “employment development office” for residents.
