Mission: Asante physician partners exists to enhance the quality and cost-effective delivery of healthcare services in the community it serves by acting as a vehicle to promote the clinical integration of physicians and hospital services.
Programs: Asante physician partners largest program service accomplishment to take place during the fiscal year ending sept 30, 2017 was to see in clinic 32,463 medicare, 23,586 medicaid, and 2,164 uninsured patients. These patients may experience difficulty in accessing care in other parts of the community. Asante physician partners stands ready to provide care for all those in need. This access is of particular benefit to the underserved in the community.
asante physician partners second largest accomplishment based upon functional expense is the recruitment and retention of providers in our community. During fiscal year 2017, asante physician partners recruited 42 providers from out of the area. One of asante physician partners primary goals is to provide to the citizens of our community a broad array of general practitioners and medical specialists. We feel this approach ensures most patient medical needs can be met locally.
asante physician partners final program service accomplishment is joining medicare's comprehensive primary care plus (cpc+) for 8 of our primary care clinics. The cpc+ model seeks to improve quality, access, and efficiency of primary care. Our participating practices continue to make strides in improving the care they provide our community through focus on care management, comprehensive care coordination, patient and family engagment with caregivers, and planning for care beyond episodes of care.
