Mission: It is the mission of Community Living Connections, Inc. to support individuals to live their best life in their home and in their community.

Direct beneficiaries per year: Serving approximately 180 adults with developmental disabilities.
Geographic areas served: Dane County, Wisconsin
Programs: Our community living programs support independent living environments for adults with developmental disabilities throughout Dane County, primarily in Madison, WI and Stoughton, WI. Our organization provides support that enables people to live in their own homes and participate in community life. We believe that individuals with disabilities deserve equal opportunities to create their "best life"; they just need additional support in order to do so. Community Living Connections provides in home staff to help with planning, personal cares, medical support, finances, relationship building…etc. We work in partnership with people throughout their lifetime.
