Mission: Access Partnership facilitates the connection between those with health care needs and available resources. Our Vision is that All residents of Hampton Roads can obtain health care needed to achieve personal wellness.
Results: Distributed donated medical supplies and equipment valued at $400,000 annually. Coordinated with Healthy Chesapeake to provide support and care-connection to residents of Chesapeake (since Jan. 2020)
Target demographics: Uninsured and under insured, low-income adults
Direct beneficiaries per year: Over 2,200 individuals served through the donated supplies and equipment program, resources and referrals, e-news and social media.
Geographic areas served: Hampton Roads Region of Virginia
Programs: The donated medical supplies and equipment program. Community referrals, outreach and education. Noteworthy Newsletter shares health care news, resources and events.
