Programs: The home health programs provides skilled nurses, physical therapists, speech therapists, occupational therapists, medical social workers, nutritionists and home health aides to homebound clients in their residence. Vna staff are specialized in wound care, certified diabetic education, infusion therapy, congestive heart failure and case management. Treatment plans are coordinated with the physician, the patient and the caregiver to help patients achieve their optimal level of functioning and independence. The program provides home assessments, patient education and skilled nursing treatments as ordered by physicians. A telehealth program is provided to remotely monitor vital signs, weight and knowledge of the disease processes for patients requiring additional support on a daily basis.
