Nonprofit Overview
Causes: Human Services, Senior Centers, Seniors
Mission: Our mission is to combat hunger and isolation in disabled and older adults by providing nutritious meals and enabling them to remain healthy and independent within their families, homes, and communities.
Results: We currently serve 1,200 meals a day to disabled and older adults all around Collin County and this past year we served 260,000 meals.
Target demographics: Adults over 60, disabled adults under 60 and veterans
Direct beneficiaries per year: over 1,500 seniors and disabled adults in Collin County
Geographic areas served: Collin County
Programs: Over 260,000 nutritionally balanced home delivered meals are provided annually to homebound seniors and disabled clients. Homebound and disabled clients are unable to shop/cook for themselves, access meals at a senior center without in-home support. All eligible clients are assessed by case managers who are licensed social workers and identified as nutritionally at risk based on teh results of scientific holistic needs assessment tool. Clients receive breakfast, lunch and weekend meals which are delivered monday - friday from 11:00 - 1:00 by volunteer and staff drivers. Volunteers pick up meals from drop off sites at senior centers and community sites. The case managers provide ongoing case management, follow-up and emergency support services to over 700 clients in collin county. Annual and mid-term assessments are conducted to determine the impact of meals upon clients' health status and quality of life.
Over 6,000 congregate meals are provided annually to seniors 60 years and older in six (6) senior centers. Nutritious lunches are provided daily between 11:00 am and 12:00 noon for seniors who are able to attend the local senior centers.
We are always in need of volunteers to help us prepare our meals, work on stocking our shelves with shelf stable items, and deliver our meals.
