Causes: Health, Home Health Care
Mission: To assist individuals and families where a physical or mental disability, handicap, chronic or acute illness, or social inadequacies threaten family living. This assistance is carried out by providing trained and supervised aide personnel whose skills in homemaking, personal care and teaching are utilized to provide a safe, wholesome home environment, to promote satisfactory family relationships, to enable people to function independently, and to offer support and assistance.
Programs: To provide home and community based services to individuals and families where circumstances threaten independent community based living. To help individuals learn skills needed to function independently in the community. To provide temporary relief to the usual caregivers and providers and provide all the necessary care that the usual caregiver would provide during the time period. To coordinate care and assistance to those over 65 years of age who qualify for the elderly waiver.
