Mission: Exceptional family resources partners with individuals with disabilities and their families, providing services, information, advocacy and other supports to enhance lives and foster community acceptance.
Programs: Medicaid services - in 2017 the agency provided its agency supported/direct provider purchased service of community habilitation to 298 individuals. It provided 252 individuals various respite services through waiver respite funding. Medicaid service coordination was provided to 199 individuals; family education and training to 10 individuals/families; supported employment to 18 individuals; pathway to employment to 25 individuals; community-based prevocational services to 21 individuals. Self- directed/broker/fiscal intermediary services to 83 individuals. Funding provided directly to individuals were: community transition services to 0, live-in-caregiver services to 3, and individual goods & services to 49 people.
in 2017 the agency provided non-waiver respite services to 39 individuals; various services through family support services: family directed supports- 180 individuals (76 for respite, 111 for goods and services-a few got both), senior caregivers-20 individuals, first step- 76 individuals, friends connection-90 individuals, and parents night out- 132 individuals; and information,referral and advocacy services to 262 individuals (not counting all those who received resource manuals). Nys 100% funding for self-directed, other than personal service provided payments for goods & services to 66 individuals.
