Mission: CCGD, founded in 1940, is a planning and resource coordinating agency for health and human service needs in Dallas and the surrounding 22 county area. Council members are 126 public and private nonprofit agencies, which have agreed to work together to assure high quality, cost-effective services, and programs for those in need.
Programs: Aging and disability services: dallas area agency on aging served and advocated for persons 60+ in dallas county. Direct service: case management, caregiver support and benefits counseling - 1,703 senior citizens and caregivers. Residential repair - 307 homes; home delivered meals - 166,647 meals; health maintenance and support - 324 contacts; chore/personal assistance - 647 hours; respite care - 7,135 hours; and legal assistance/awareness- 8,302 persons. Allocated federal funds to sub-grantees for services provided to senior citizens and caregivers including 332,011 congregate meals at 25 senior centers, transportation - 107,440 trips; caregiver information services - 60 events, instruction, training, and nursing home ombudsman activities - 2,730 contacts. Ctn provided mobility management services to over 1,340 people.
information and assistance:provided comprehensive information & referral assistance to individuals calling 2-1-1 in collin, dallas, denton, ellis, hunt, kaufman, navarro & rockwall counties. 609,368 calls were answered and responded to. Over 25,000 people were given information concerning available services & benefits at community health fairs and events. 1,500 copies of the annual source book were sold.
navigator project:to provide health insurance market place navigator services in an 18 county area. 56,245 individuals were educated/assisted with affordable care act enrollment.
