Causes: Community Health Systems, Health
Mission: Improve the quality and efficiency of healthcare delivery for, and the health outcomes of, community members in the columbia gorge region, which may include all or a portion of hood river and wasco counties, oregon, and adjoining counties. Govern the coordinated care organization serving the region, bring public and private stakeholders together to develop and implement collaborative community health initiatives to improve coordination, integration, prevention, accountability, elimination of disparities and lower costs for healthcare services in the region.
Programs: 1) completion of the first regional health assessment which included 6 counties, > 1000 community surveys and 2 focus groups. The regional health assessment required convening 11 local organizations listed below. This comprehensive document not only met state and federal requirements, it provided a basis for several grant applications since its completion in late 2013. Local organizations have been able to use this document for grant-writing purposes. According to peers, this appears to be the most collaborative health assessment completed in oregon. Our work has not concluded, but the designation by the governor highlighted the collaborative approach.
