Mission: To promote and carry out community based services to frail, disabled or elderly individuals in the community and to develop models of affordable and effective in-home care for the frail or disabled living in the community and to promote collaboration and coordination among long-term care and senior housing providers.
Programs: The organization renovated and expanded a senior residence to increase the number of residents it can serve and added a patient care-centered medical center. The majority of the space is leased to mount pleasant home to operate the senior housing component and the medical center is leased to a local hospital. The organization will provide in-home care and provide other community services to the residents of the senior housing facility. The residence will serve as a model of affordable and effective in-home care for the elderly in the community.
