Mission: Ccih provides permanent affordable housing coupled with well-executed support services to people who are homeless or at risk to become homeless in contra costa county. Support services are individualized for each person or family in order to assist them to stabilize their health and housing and become as self-sufficient as possible. Adult services focus on mental and physical health, life skills, employment, parenting, and eviction prevention. Youth services focus on emotional and physical health, education, social and life skill building, good citizenship and preparation for adulthood. All services are accessible in that they are provided on-site at apartment complexes or in consumers' homes.
Programs: Ccih provides permanent, affordable housing and on-site support services to formerly homeless and at-risk families and individuals. Supportive services programs assist families and individuals to stabilize in housing improve self-reliance. Services include curated case management to fit the needs of every household, youth afterschool academic support and enrichment, life skills building (like budgeting and parenting), mental health counseling from licensed mental health professionals, and continuing education and employment support. In the last fiscal year, ccih served 1,100 people.
in the last two years, ccih expanded its scattered site masters lease model. Ccih houses 48 formerly chronically homeless individual adults (many of whom are seniors) with special needs. Additionally, ccih also housed 21 formerly chronically homeless families in the same scattered site model. A mobile service team of case managers provide services to ensure their stability, physical and mental well-being, and to assist them in retaining their housing and increasing life skills. The children in this program receive intensive academic support, and have access to mental health counseling.
ccih provides housing advocacy services for contra costa county residents diagnosed with hiv/aids. Advocacy includes emergency housing, housing location, rental assistance, ongoing monitoring of housing needs, case management, and eviction prevention support. During this period, 136 unduplicated clients were served.
last year, ccih partnered with contra costa county to launch and implement the county's housing navigation program which is part of the county's coordinated entry system and creates "one door" for homeless families and individuals to gain access to affordable housing and appropriate services. Through the county's housing navigation program, ccih conducts housing search and placement for those seeking housing, targeting resources and supports to best meet the needs of the individual.
