Mission: Touchstone mental health is an organization of excellence creating innovative, person-centered care with individuals whose lives are affected by mental illness. We transform how services for mental, physical and social health intersect to achieve optimal wellness.

Programs: Touchstone's rising cedar apartments is a unique, 40 one-bedroom unit assisted living apartment building offering state-of-the-art construction specifically designed as a healing environment to enhance health and wellness for adults. Rising cedar apartments offer co-created mental health services such as illness and symptom management skills assistance, independent living skills assistance, medication administration, health care coordination and more. The program on average housed 37. 6 residents per month which represents a capacity utilization of 94%. And for 2017, residents surveyed had a satisfaction rate of 95% for both services and staff. Assisted living, new hope is minnesota's first long-term assisted living program for adults 18 years of age and older with a serious and persistent mental illness. This program provides residents with 24-hour supportive services so they can enjoy life's opportunities. Assisted living, new hope, on average housed 17. 4 residents per month which represents a capacity utilization of 90%. And for 2017, 100% of the residents surveyed were satisfied with services and with staff.

touchstone residential treatment offers intensive residential treatment services to individuals with a serious and persistent mental health diagnosis, who may also be struggling with substance use disorders. Treatment services are provided up to 90 days, 24 hours a day, 7 days a week. This program provides a structured, supportive and comfortable environment focusing on integrative services including mind, body and spirit. In 2017, the 14 bed residential treatment program in minneapolis served 80 clients. A second residential treatment facility was opened in bloomington in october 2015 with a 16-bed capacity. In 2017, 87 clients were served. In november 2017, a third residential treatment facility was opened with residential treatment and crisis beds. In 2017, 60 clients were served.

touchstone care coordination provides face-to-face, telephonic and collaborative support starting with a comprehensive, client-centered assessment highlighting strengths and needs. This program provides direct assistance from a care coordinator with health care navigation, along with social service supports. In 2017, 838 clients received coordination services.

touchstone targeted case management services provides resources for adults to maintain and increase independence, minimize the risk of hospitalization, and improve overall functioning and well-being. This program offers support with the creation of a treatment plan to identify individual goals and develop strategies for goal achievement, while using an integrated approach to health and wellness. In 2017, 382 clients received case management support. Touchstone intentional communities individuals (community members) came together as a supportive group, sharing common purpose and working cooperatively to create lifestyle reflecting shared values. Community members are able to maintain housing stability, reduce the risk of hospitalization, gain independent living skills, and build social supports by becoming a contributing member of a community. In 2017, the program maintained 100% of capacity while 7 people graduated the program, so the program expanded to serve an additional 5 people for a total of 45 people being served at one time moving forward. Touchstone's home & community based services offers long-term stable housing with supportive services to adults living with a mental illness. This program provides a combination of consumer-directed services focusing on promoting independence, community integration and successfully transitioning individuals into independent community living. The program has grown to have housed 41 people into stable housing by year end 2017, significantly ahead of the program goal. For 2017, 100% of residents surveyed expressed satisfaction with socialization and community integration opportunities provided through a collaboration of both staff support and client input.
