Mission: We, st. Peter's health partners and trinity health, serve together in the spirit of the gospel as a compassionate and transforming healing presence within our communities. Founded in community-based legacies of compassionate healing, we provide the highest quality comprehensive continuum of integrated health care, supportive housing and community services, especially for the needy and vulnerable. The community hospice is a member of st. Peter's health partners and trinity health.
Programs: The community hospice, formed in 1997, is located throughout eight counties in eastern new york state. When people are in the final stages of life, the community hospice helps them to live with dignity, without pain, and in familiar surroundings. The community hospice provides comprehensive comfort-oriented services and compassionate attention in patients' homes, in the hospital, at nursing homes, or a hospice residence. A person is eligible for hospice care if their physician estimates a life expectancy of six months or less. Patients can expect regular visits from a caring team of professionals that includes nurses, social workers, chaplains, and home health aides. A plan of care is developed for each individual based on the particular needs of the patient and family. In addition, the community hospice will provide the patient's family and loved ones with the support they need, including grief and bereavement programs.
