Causes: Centers to Support the Independence of Specific Populations, Homeless & Housing, Human Services
Mission: As lead agency for providers of homeless/prevention services in manatee/sarasota, we plan strategies with partners, assist with grants, educate the public, advocate with local leadership, administer the management information system to assure quality data and case management.
Programs: Pass through homeless assistance:the partnership provided funding to direct service providers through three assistance programs, (1) tanf homeless prevention assistance in the amount of $ 48,650 (2) state of florida challenge grant assistance in the amount of $ 148,500 and (3) state of florida emergency solutions grant assistance in the amount of $ 270,819 for a total of $ 467,969.
hmis:the homeless and at-risk management information system (hmis)is a foundation for collaboration and delivery of services. The program staff monitor the system activities, provide training for users, assist agencies with workflow design, run customized reports to support grant requirments, review data for quality and timeliness, provide help-desk support to users, and work closely with the hmis committee to develop policy and implement improvements. This program served 28 provider agencies and supported over 200 users.
continuum of care:the continuum of care program activities are described in both federal and state law and require the lead agency to convene regular meetings to coordinate services, provide education and find solutions to local homeless issues. Annually, we develop a collaborative application to the department of housing and urban development, with the prior year application bringing $724,526 directly to providers. Every january, we coordinate a regional homeless census, which in turn provides demographics on local homeless, and a picture of the complexity of the problem. We have worked closely with sarasota county and dr. Marbut on surveys and plan development. Outreach and education is achieved through public presentations, our website, e-newsletter, facebook page, fact sheets, and resource guides provided to help those homeless and at risk find services. 95% of the funds we receive go directly to program services.
