Mission: The schc clinic and neighborhood transformation proposals work together to utilize the assets of the neighborhood to meet the challenges of the neighborhood by providing ongoing education and training programs as well as comprehensive primary health care programs to improve the quality of life for residents in the community through neighborhood health care services and employment opportunities.
Programs: The schc clinic offers affordable health care for individuals regardless of income, insurance, or ability to pay. Schc clinic provides care for a person: physically, psychologically, socially, and spiritually. The schc clinic provides prevention and education programs that emphasize wellness. In 2016 the schc clinic saw 3,846 patients.
the neighborhood transformation (nt) program provides a sustainable initiative that engages residents to address their own health and social needs. Nt helps residents empower other residents to improve their living situation by providing an asset-focused approach for the neighborhood. Nt partners with organizations, businesses, churches, and families to solve problems and address concerns in the neighborhood.
