Provided transitional case management services to 69 young adults between the ages of 18-30 who are living with a mental health diagnosis.
in 2017, the agency continued the early childhood initiative, which is a voluntary home visiting program in madison/dane county, wisconsin, for pregnant women and families with children aged 0-4. Early childhood initiative services are grounded in the knowledge that families are more successful when parents and children have access to responsive, comprehensive and community-based support. During 2017, rise wisconsin, inc. Worked with 121 families through the early childhood initiative program.
