Programs: Coordination and administration service programs- these coordination and administration services are provided to governmental agencies and other not-for-profit organizations and are primarily centered around health management staffing, as well as intermediary services which oversee subcontracting to agencies providing direct services, and monitor quality and financial processes. Management services entail managing contracting and subgranting processes, monitoring program quality, developing and maintaining data systems and tracking outcomes, providing and coordinating professional development and training activities, supporting provider networks to create learning communities and improve programs, and efficiently overseeing payments to provider agencies. Phmc administers the out-of-school time project on behalf of the city of philadelphia, serving as the intermediary for more than 80 community-based organizations offering high quality after school and summer programs in public, parochial and charter schools and community sites to 18,000 elementary, middle and high school students. Phmc also provides direct out-of-school time programming through two 21st century community learning center sites for middle school students in philadelphia. Phmc serves as the southeast regional key ("serk"), on behalf of the commonwealth of pennsylvania, administering the early childhood education quality improvement system for the office of child development and early learning, supporting 1600 early learning providers and reaching 60,000 children in southeastern pennsylvania (philadelphia, delaware, bucks, chester and montgomery counties). Phmc also serves as the intermediary for the parenting collaborative, on behalf of the city of philadelphia, overseeing more than 40 agencies providing evidence-based parenting education services.
integrated health services (formerly behavioral health and specialized health services)- phmc's behavioral health programs provide trauma-informed, evidence-based residential, intensive outpatient, outpatient, partial hospitalization and case management services to individuals with substance abuse and/or mental health issues. Programs serve low-income women, women with children, adolescents, individuals involved in the criminal justice system and adults who are dually-diagnosed with substance abuse and mental health issues. Treatment addresses the psychological, emotional, physical, educational and spiritual aspects of addiction, mental illness and abuse. Phmc provides primary care, case management and supportive services to the homeless populations in shelters and to persons in transitional housing in the region. In addition, phmc operates a network of six full-service community-based federally qualified health centers employing an innovative nurse-managed model, which serve to increase access to high quality primary care for at-risk community members. Phmc's health centers provide integrated behavioral health services and encompass specialized services for community members with hiv/aids and hepatitis c, and for homeless community members, many of whom are seniors or veterans, and most of whom have complex healthcare needs. Phmc also operates philadelphia's first medical respite care program serving homeless residents released from hospital stays following surgeries or other treatments who require intensive support to prevent unnecessary hospital readmission. Phmc administers hiv-aids prevention and services programs, including community health outreach, which conducts health promotion services for individuals at high risk of hiv and hepatitis c infection and transmission. Outreach staff engage with substance users in individual street level encounters and group venues such as homeless shelters and feeding programs, and provide hiv/hcv risk reduction education, counseling and referral to substance abuse, hcv screening and hiv early intervention services.
phmc provides service coordination through the childlink program for families of infants and toddlers who have development delays. Service coordinators help the family access early intervention services and community supports to address the needs of their child. Service coordinators also provide periodic developmental screenings to children at risk of developmental delay. Phmc administers personlink, which provides supports coordination in philadelphia, montgomery, chester and delaware counties to individuals with intellectual and developmental disabilities. Supports coordinators secure services that allow individuals to live at home and participate in their community. Supports coordinators develop a service plan and monitor service delivery to ensure the individual's health and safety. The personlink special offenders unit is a highly trained team that provides supports coordination to individuals with intellectual and developmental disabilities involved with the criminal justice system. The special offenders unit offers assistance to the courts to link individuals with intellectual and developmental disabilities to placement and services in the community. The special offenders unit provides awareness, education and supports to help these individuals and their families navigate the criminal justice system and work toward eliminating continued criminal involvement. Phmc serves as the case management agency for hud's north central philadelphia choice neighborhood program. The objective of the program is to improve educational outcomes and economic self-sufficiency for public housing residents. Through its affiliate turning points for children, phmc operates two community umbrella agencies (cuas) in philadelphia as part of the department of human services' improving outcomes for children initiative. These cuas provides a continuum of care for children and families, including services preventing child abuse and neglect, in-home services for families at risk, and foster care services aimed at reunifying families and assuring permanency for children.
health promotionphmc administers a broad range of health promotion programs and services focusing on disease management, tobacco cessation, asthma and lead poisoning prevention, traffic safety, adolescent sexual health and pregnancy prevention, nutrition, active design and other health and wellness initiatives. Educationphmc administers head start programming through the philadelphia head start partnership ("phsp"), providing head start services to more than 600 children, ages three to five, and families in southwest philadelphia. Phsp delivers these services through direct operations in southwest philadelphia and through partnerships with high-quality childcare centers, charter schools and a family childcare network in the region. As a head start administrator, phsp offers comprehensive programs that support physical, mental, social and emotional development of children from low income families to nurture future success in kindergarten and beyond. Phmc offers workforce programming to educate and train individuals who have limited access to resources, to connect them to employers and help meet the changing needs of the workforce. Phmc's workforce programming encourages sustainable careers and economic independence by building a supportive learning environment in which students receive personalized attention. Other professional and technical assistance activitiesphmc provides management services for local, state and federal government entities and back-office services and strategic support to affiliate organizations. Back-office support includes accounting, human resources, communications, facilities and other administrative services. Research and evaluation group at phmcphmc's research and evaluation group is committed to utilizing rigorous research and evaluation methods to illuminate today's most critical public health problems. Research and evaluation are the backbone of good public health practice, creating a strong foundation for the design and delivery of programs and services that can effectively prevent disease and improve health. The research and evaluation group supports phmc and affiliate programs to use evaluation data to guide and continually improve program effectiveness, and to measure program impact. In addition to supporting phmc and affiliate programs, the research and evaluation group is engaged in public health research and program evaluation with a diverse portfolio of external clients and funders. The research and evaluation group houses the community health data base's household health survey, the largest, local public health survey in the country. Phmc is also a partner with the delaware public health institute (dphi) which houses the state health improvement plan and de household health survey. In addition, the research and evaluation group has expanded over the last year to include two additional capacities by merging with research for better schools and treatment research institute.
