Causes: Human Services, Residential Care & Adult Day Programs
Mission: Our mission is to provide exceptional care and service giving residents, families and visitors the peace of mind they need and desire. At every level, Notre Dame Health Care gives renewed meaning to the words family and community. We offer expertise in elder care, memory support, rehabilitation and end-of-life care serving 230 residents with assisted living and skilled nursing, and more than 400 adult and pediatric hospice and palliative care patients in the community each year. Our Educational Bridge Center fosters a culture of learning and growth, where individuals are empowered and provided guidance and support to attain their personal, academic and professional goals. We assist healthcare workers and adults to enhance skills, advance careers, and broaden horizons, leading to higher wages and increased stability within families. In addition to our employees, 175+ students from the community currently have access to our computer lab, career counseling, and tutoring services.
Programs: Notre dame long term care center/notre dame du lac/hospice - provide skilled nursing home care to the elderly and disabled. Notre dame long term care center/notre dame du lac/hospice - pedipal program - provides home care to children with life limiting illnesses.
